This selection follows the successful completion of the 1st and 2nd rounds of counseling.

- Download the PDF and search for your name/roll number
Required Documents
At the time of admission, candidates must deposit the following original documents:
- Admit Card of HSLC & HSSLC (or equivalent).
- Mark Sheets & Pass Certificates of HSLC & HSSLC.
- Caste Certificate (if applicable).
- EWS Certificate (if applicable).
- Permanent Residential Certificate (PRC).
- Medical Fitness Certificate: Issued by the Joint Director of Health Services of the candidate’s resident district.
Important Note on Withheld Seats & PRC
- PRC Submission: Candidates whose seats were withheld during counseling due to the non-submission of the PRC must submit the document to the office of the Director of Health Services (FW), Assam by 24.12.2025.
- Consequence of Failure: If the PRC is not submitted by the deadline, the seat will be cancelled and considered for subsequent rounds of counseling.
Medical Fitness Requirement
Admission to the ANM Training Course is strictly subject to medical fitness.
- Candidates must submit a Medical Fitness Certificate
- Issued by the Joint Director of Health Services of the respective district
- Only medically fit candidates will be finally admitted
Reporting Date & Admission Process
All provisionally selected candidates must personally report to the Principal of their respective ANM Training School along with all original documents.

If a candidate fails to report within the given date, their seat will be cancelled automatically
